Case updates
・ As of 20 May 2026、コンゴ民主共和国からエボラウイルス病 Bundibugyo virus disease (BVD) による 64例 confirmed cases and 6例 six confirmed deaths, 671 suspected cases, and 160 suspected deaths が報告されている。1261例の接触者が確認されている。
・ 発生地域 Affected healthzones in Ituri include Mongbwalu, Rwampara, Bunia, Nyankunde and Bambu. Affected health zones in Nord-Kivu include Goma, Katwa, and Butembo.
・ South Kivu Province において5月20日、 the Miti-Murhesa health zone near Bukavu で初めての確定診断例 its first confirmed case of Bundibugyo virus disease が確認された。Kisangani in Tshopo Province の28歳の患者で、検査確定前に死亡した。もう1名の有症例が隔離され検査が行われている。
・ 確定診断例が確認された地域は、以前からの Bunia, Goma, Butembo, Kampala に加え、新たに South Kivu Province に拡大した。
・ 5月20日の Western Equatoria で疑い例の確定検査が行われているとの報道に対し、南スーダン政府当局は no case was detected と述べている。
Epidemiological analysis
・ アウトブレイクはコンゴ民主共和国東部で地理的な拡大が続いており、 Ituri, North Kivu, and South Kivu provinces で感染例が確認されているのに加え、輸入感染例 imported transmission がウガンダで報告されている。
・ 人口密度の高い都市部や紛争地域での感染確認が、サーベイランス、接触者追跡、隔離、感染予防対策が困難となっている。
・ 資材不足などがあり、現在も医療現場での感染伝播が大きな懸念となっている。
Vaccine and therapeutics updates
・ WHO advisors は特異ワクチン Bundibugyo-specific vaccines の開発・評価に数か月を要するとしている。investigational candidate vaccines and therapeutics remain under technical review.
・ Laboratory findings では Bundibugyo ebolavirus に対する効果の可能性 potential activity against が報告されているものの、実験段階にあり、臨床試験を経ていない。
・ The Filovirus Collaborative Open Research Consortium (CORC), led by The French National Research Agency for HIV, viral hepatitis and emerging infectious diseases (ANRS MIE), together with Africa CDC and the WHO R&D Blueprint, are convening an urgent scientific consultation on 22 May 2026 to review and align research and development (R&D) priorities for medical countermeasures (MCMs) against Bundibugyo ebolavirus. South Sudan conducts additional tests to confirm first suspected case of Ebola
The government of South Sudan により、5月18日にコンゴ民主共和国国境に近い Western Equatoria state から報告されたエボラウイルス感染が疑われる患者 a suspected case of the Ebola virus について、確定診断のための追加の検査 additional laboratory tests が行われていると、当局者の話で明らかになった。director of Epidemiology at the South Sudan Ministry of Health が "We are currently conducting further investigations into the case reported by the state of Western Equatoria," と語った。

ペニシリン耐性B群連鎖球菌 A penicillin-resistant strain of Streptococcus agalactiae (group B Streptococcus, GBS) が確認された。他剤にも耐性 also multidrug-resistant (MDR)となっている。GBS のペニシリン耐性は多くはないが、世界的に報告が増えている。同菌は、ペニシリン結合タンパクの特異的な遺伝子変異が認められている。

Penicillin is the first-line agent for both prophylaxis and treatment of Streptococcus agalactiae (also known as group B Streptococcus, GBS) infections .
Discussion
Group B Streptococcus infections は消化管と泌尿生殖器の共生菌 a commensal ではあるが、ヒト及び動物の感染が報告されており、新生児など very young では侵襲性疾患となり得る。予防及び治療の第1選択薬であるペニシリン penicillin の耐性の報告はまれではあるが、世界中で PRGBS（ペニシリン耐性GBS）の報告が増加していることから、the 2024 World Health Organization (WHO) Bacterial Priority Pathogens List リストに入れられた。
イタリア初の the first case of invasive GBS infection caused by a PRGBS strain の報告であるが、ヒトの感染例の報告はボランタリーベースで行われている。
過去にも多くの報告があるとおり、今回の菌は多剤耐性菌でもあるが、過去報告された PRGBS strains は、慢性人工関節の感染症 chronic prosthetic joint infections により長期間抗生物質による治療を受けていた成人 older adults から分離された株である。このような症例により、長期間の抗生物質選択圧による変異リスク the possible risk of point mutations accumulation が明らかになった。
今回報告された菌株は serotype Ia/ST23 である。他の報告もPRGBS serotype Ia が報告されているが、the genetic lineage が特定されていない、または different serotypes/lineages であった。Serotype Ia/ST23 PRGBS は米国とドイツで報告されているが、耐性の表現型に重要と考えられている the pbp2X and pbp2a genes 変異が今回の分離株とは異なっていた。
Conclusion
Here, we describe the first case of invasive infection caused by a PRGBS strain in Italy, which was also MDR.
